Paying Premiums
If I elect to pay through monthly automatic debit or credit card withdrawal, when will my premium be deducted from my bank account each month?
The withdrawal transaction will occur on the 19th day of the month (or the next business day thereafter) immediately preceding the month for which the premium is due.
Will Delta Dental accept premium payments from anyone other than the subscriber?
Delta Dental will accept an annual payment from the subscriber or another individual, such as a friend or family member. Monthly automatic debit or credit card withdrawals can only be processed from the bank account of the individual applying for coverage. Payments from a corporate account cannot be accepted.
Can I make a change to my payment method?
At renewal, you have the opportunity to change your method of payment or continue your current method of payment. You can change your method of payment by logging in to your account or by completing Section V, VI and VII of the Renewal/Change Application.
What do I do if I want to change my bank account used for the automatic withdrawal of premiums?
You can change your bank account information by logging in to your account or by completing Section VI and VII of the Renewal/Change Application.
Coverage Outside of Missouri
Do I have coverage outside of Missouri?
Yes, your Delta Dental coverage travels with you. Common examples are:
- A part-time secondary residence outside of Missouri
- Traveling outside of the state of Missouri, including international travel
What if I permanently move out of Missouri?
Delta Dental of Missouri’s individual and family plans are only available to permanent Missouri residents, and you will need to cancel your coverage if you are moving out of the state. Please notify us of your move in writing. You will be asked to show a utility bill or a lease document containing your name and your out-of-state address in order to terminate the plan. Your coverage will terminate at the end of the month in which you change permanent residency or provide notification to Delta Dental of Missouri, if later.
I am moving out of Missouri and need to cancel my membership. Will I be reimbursed for any premiums that I paid in advance?
Your coverage will terminate at the end of the month in which you change permanent residency or provide notification to us, if later. You will receive reimbursement for any remaining premiums you paid in advance.
My dentist practices in Missouri but I do not reside in Missouri. Can I enroll in Delta Dental Individual and Family benefits if I live outside of Missouri?
Delta Dental of Missouri’s individual and family plans are only available to permanent Missouri residents, no matter where their dentist practices. Residents from other states are not eligible. If you live outside of Missouri and you are interested in individual coverage, you can visit Delta Dental Plans Association's individual coverage locator to review the options in your state.
Plan Coverage
Are there waiting periods for coverage?
There is no waiting period for diagnostic and preventive services under these plans. For Bronze, Silver and Gold plans, there is a 6-month waiting period for fillings, and a 12-month waiting period for major services, including crowns, root canals, bridges, periodontal surgery and dentures. For Gold plans there is also a 12-month wait on implants. For Platinum plans, there is no waiting period on fillings and a 12-month waiting period for major services including crowns, root canals, bridges, periodontal survey and dentures. The Platinum plan also requires a 12-month waiting period for implants. Waiting periods will be waived with proof of 12 months of continuous coverage, under an insurance contract with similar dental coverage for fillings and major services, which ended within 63 days of your plan’s effective date.
What do I need to prove I had 12 months of coverage for restorative and major services with my prior dental carrier?
Send Delta Dental a statement from your previous insurance carrier, stating your coverage period (effective date and termination date) and that you have had 12 months of prior continuous coverage for yourself and applicable dependents. The policy must have included coverage for fillings and major services, which ended within 63 days of your plan’s effective date. This statement can be mailed to Delta Dental, 12399 Gravois Road, St. Louis, MO 63127 or faxed to Delta Dental at 314-656-2900. Please include your name, address, phone number and “Individual and Family” on the front of the document.
When is a "pre-existing condition" not eligible for prosthodontic coverage?
A pre-existing condition in prosthodontics would occur when you have one or more missing teeth, lost before your coverage under Delta Dental Individual and Family begins. Benefits under this plan will not be available for the initial replacement of these missing teeth.
Eligibility and Coverage
When will my dental coverage begin?
All complete applications received on or before the last day of the month will be effective the first of the following month, provided the application for coverage is accepted. For example, a completed application received September 15 will be effective on October 1, provided the application is accepted (e.g., you satisfy the eligibility requirements, the application is properly completed and the premium is received). Members purchasing their plan online directly from Delta Dental have the option to delay their effective date by three months.
Can I terminate my coverage before my 12-month coverage commitment has expired?
This program has a 12-month commitment. You cannot voluntarily withdraw from the plan until the 12-month commitment period has expired, unless you are no longer eligible for coverage. If you move out of Missouri, please provide a utility bill or a lease document containing your name and your out-of-state address, and we will terminate the plan. Terminations will be effective at the end of the month in which you provide notification to Delta Dental.
What if I get a job that offers benefits while I'm covered by Delta Dental Individual and Family? Will I be able to cancel my membership?
This program has a 12-month commitment. You cannot voluntarily withdraw from the plan until the 12-month commitment period has expired, unless you are no longer eligible for coverage.
When can I add or remove a spouse or dependent?
You can add or remove a spouse or dependent annually on your anniversary month or within 31 days of a qualifying event, such as marriage or adoption. Effective dates for membership changes follow the schedule stated for initial effective dates (see "When will my dental coverage begin?"). To add or delete a spouse or dependent, log in to your account or complete Section III of the Renewal/Change Application.
When will I receive my ID card?
You will receive two ID cards in your welcome kit, which will arrive two weeks after your Delta Dental Individual and Family plan application is accepted. If you need a temporary ID card before you receive your welcome kit, log in to your account to download a temporary card.
